Intake form

The intake form.

It captures only the structured facts. Nothing the call is better at. Nothing we can crawl ourselves. It lives in two places: business identity rides on the order form at checkout, and the rest is a short page right after they pay, while they are still in signup mode.

Section 1 · On the order form (checkout)

Captured during the purchase. We customize the order form to collect these, so they are done before they ever see the intake page.

Right after checkout: "Great, you're in. Fill out these few things." They fill as much as they can, then submitting it drops them on the thank-you video page, so it still feels like part of signup.
